BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2014203731 DESCRIPTION We present a case of a 63-year-old woman with hypertension and diabetes mellitus, who was admitted for left lumbar pain, fever (38.5°C) and nausea. Abdominal ultrasound revealed several heterogenous splenic nodules and an enlarged left kidney with a 7 mm calculus in the medium pyelocaliceal system. Contrast enhanced ultrasound showed multiple simple splenic cysts and a nodule in the inferior pole, with rapid arterial peripheral enhancement (figure 1A), enhancement in the late phase (figure 1B) without washout, with irregular contour, inner septum and possible central necrosis. Abdominal CT showed a 21 mm, heterogeneous, hypodense nodule in the inferior splenic pole (figure 2A), and inflammatory densification of perirenal fatty tissue with thickening of the perirenal fascia (figure 2B). Biologically, the patient presented with leukocytosis, hyperglycaemia and inflammatory syndrome. Urinary cultures were negative; blood cultures grew Streptococcus spp. The patient received antibiotic treatment and the symptoms and ultrasound aspect improved in about 2 weeks. We conclude that the patient presented splenic abscess due to acute pyelonephritis. Diabetes is a risk factor for this rare condition; splenic abscesses may appear by haematogenous metastasis or contiguity. Other cases showed improved outcome by splenectomy; in our case this was not required, as the patient’s evolution was satisfactory under antibiotic treatment. Although there are no pathognomonic symptoms, in patients with high suspicion of infection who present with abdominal pain, fever and nausea, splenic abscesses are diagnoses to be considered.

Xanthogranulomatous pyelonephritis successfully treated with antibiotics only.

A 73-year-old woman with leukemia presented with urinary tract infection, splenic abscess, and a renal mass. Both urine culture and pus culture of the splenic abscess yielded Klebsiella pneumoniae. Percutaneous biopsy of the renal mass confirmed the diagnosis of xanthogranulomatous pyelonephritis. Because of high risk for surgery, the patient received treatment with antibiotic therapy for 2 mon...

Acute pyelonephritis including an overlooked renal cell carcinoma

Hidden renal mass is not evident during severe inflammation such as acute pyelonephritis, pyonephrosis, or renal abscess. Deceptive inflammatory presentations that can occur in aggressive synchronous renal malignancies can significantly delay the diagnosis of cancer. Herein, we report a case of acute pyelonephritis involving overlooked renal cell carcinoma.

High-grade transitional cell carcinoma masquerading as a xanthogranulomatous pyelonephritis and perinephric abscess

Xanthogranulomatous pyelonephritis (XGPN) is an atypical long-term pyelonephritis with destruction of renal parenchyma and a long-term inflammatory infiltrate of macrophages. Reported presentations of transitional cell carcinoma (TCC) are different. A 73-year-old woman presented with loin pain, prostration, and fever. Computed tomography scan revealed poor cortical enhancement of the kidney, bu...

Spinal epidural abscess caused by group B Streptococcus in a diabetic woman presenting with febrile low back pain.

Because spinal epidural abscess is usually ignored in the preliminary differential diagnosis of low back pain, appropriate treatment may be delayed. Adult spinal epidural abscess is sparsely caused by the pathogen known as group B Streptococcus. In this paper, we report the case of a diabetic woman with lumbar epidural abscess and vertebral osteomyelitis caused by group B Streptococcus. Owing t...

Splenic Infarction in a Case of Acute Promyelocytic Anemia

Splenic infarction occurs due to occlusion of splenic vessels that leads to splenic tissue ischemia and necrosis. There are several reports regarding splenic infarction in patients with acute myelogenous leukemia (AML). Herein, we report a case of acute promyelocytic anemia (AML-M3) who presented with abdominal pain and splenic infarction.
